CHS Winter Sports Registration & Meeting

Registration Night & Mandatory Sports Meeting
Wednesday, November 7, 5:30 PM

Welcome to Lancer Winter Sports!

All parents with students hoping to participate in a 2012 WINTER sport or club must register at Churchill on Wednesday, November 7, in the Auditorium, 5:30 PM.

Winter Sports Options:

Boys Basketball ($200) *** Girls Basketball ($200) *** Swimming ($150)
*** Wrestling ($200) *** Dance ($100) *** Cheer ($100) if brand new ***
Additionally to sports fee above, all athletes must pay annual $30 ASB fee

Registration Night Requirements:

  • Download and properly complete/sign all registration forms (Forms available HERE) .
  • All fees must be paid by November 7 , or a payment plan contract signed and approved, *** This includes “Try-Out” sports. *** ( Payments for sports requiring try-outs will not be processed right away. Following try-outs, any payment by a student making the team will be processed; payments by students not making the team will be returned. Please leave money orders blank.) *** Fees only may be paid early at the Churchill Finance Office, prior to Nov. 7 th . (At this time, fees may be paid by check/money order or cash only.) All other registration paperwork cannot be processed early , and must be submitted by a parent at the Nov. 7 th Registration Night.
  • Each student must have a current sports physical on file that is good through Mar 20, 2013 . (You may call the Health Center at 541-790-5227 to check if your student’s physical is up-to-date.)
  • Each student must be covered by insurance, with a group policy number noted. Students are NOT eligible for practice or try-outs until insurance is secured with a policy number! School insurance policies are available, but may take up to two weeks to process. Call the office at 541-790-5100 for more information…
  • Each student must have passed at least five classes at the end of the 2011-12 2 nd Semester to be eligible, and must be enrolled in and passing four classes currently. Grades will be checked at Registration Night.
  • Fee Reduction Policy (Free & Reduced Meal Status): In order to qualify for the Sports/Club Fee Reduction at registration, students must present yearly documentation verifying their Free & Reduced Meal status. ***If there is no documentation at registration night, students are required to pay in full.***
  • Apply Online (Faster process!)… You may complete an online Free & Reduced Meal Application at: https://district.ode.state.or.us/apps/frlapp/default.aspx.
  • Apply by Mail – Print and complete the attached documents… REMEMBER! It takes up to ten days if applying by mail for Free & Reduced Meal status for documentation to be mailed home, so don’t delay!

(CHS Finance Office will Be Closed Thursday, November 8, for sports processing.)
