Back-to-School Registration and Enrollment 2017

Enrollment

New students should try to complete enrollment before registration day. Our registrar is available during our summer office hours, Monday–Thursday, 9:00 a.m.–12:00 p.m., 1:00 p.m.–3:00 p.m., closed on Fridays. Enrollments will be accepted on registration days, but we encourage you to enroll ahead of time to help your registration day run smoothly.

You may pick up enrollment paperwork at the office or print it HERE (scroll to the bottom of the page) and bring it to the office. You will also need your state-issued photo ID, two proofs of address, immunization record, and previous high school transcript (if you have one). Refer to the enrollment forms for a list of acceptable ID and proof of address.

Registration

All students are expected to attend registration. Students will receive class schedules, pay fees, take school photos and get student ID cards, buy parking passes, turn in free/reduced meal forms, sign up for lockers, and more. (Note: all students receive a student ID card free of charge. Students who choose to purchase an ASB card will receive an ID card with an ASB seal.)

  • New Student, 9th and 12th Grade Registration is Thursday, August 24, 9:00am–12:00 p.m.
  • 10th and 11th Grade Registration is Friday, August 25, 9:00 a.m.–12:00 p.m.

If you or your student are unable to attend their assigned registration day, you may attend on the alternate day. If no family member can attend, please contact the main office at 541-790-5100 or come in during office hours to make other arrangements.

Please complete the sheet and send it with your student, along with the appropriate amount of cash or check to match the sum of required fees and optional charges.

Athletics Fees will not be accepted at Registration. Athletic fees must be paid prior to participation in athletic practices on August 14.

We encourage all families to pay fees at the office PRIOR to Registration. Bring your pink fee receipt with you to Registration to skip the finance station and proceed much more quickly. Finance Office hours are Monday–Thursday, 9:00 a.m.–12:00 p.m., 1:00 p.m.–3:00 p.m., closed on Fridays.

Parking Contract 17-18

ALL MOTOR VEHICLES PARKED ANYWHERE ON THE CHURCHILL HIGH SCHOOL CAMPUS MUST DISPLAY A CURRENT CHS PARKING HANG TAG (violators will be ticketed). Please be aware that students violating the parking contract and/or regulations may be ticketed, and may have parking privileges revoked at any time.

Student ID and School Portraits

All students must take a photo and receive a student ID card free of charge. Students who choose to purchase an ASB card will receive an ID card with an ASB seal. Go to www.mylifetouch.com and enter the Picture Day ID “LO437727Y0” to order your school portrait package.

An Important Note to Parents About Education Records and the Release of Directory Information

School Information Available for Noncustodial Parents

The Eugene School District is able to provide all co-custodial or noncustodial parents a duplicate copy of all district labeled mailings. Please let the school office know if you would like the name of a co-custodial or noncustodial parent added to the student’s enrollment information.

Hours of School Supervision

It is important for you to be aware of the hours that there is no supervision of buildings or grounds at Churchill High School. The school provides supervision only between the hours of 7:30 a.m. and 4:00 p.m., and during official school events and programs that occur before and after school.

School to Inform Parents if Their Student Receives a Threat

It has always been our practice to keep parents informed if their son or daughter receives a serious threat of violence or harm at school, but it is important for you to know about procedures we have implemented to comply with recent changes in State Law.

Oregon law requires schools to inform parents if their son or daughter’s name appears on a targeted list (i.e., a “hit list”) that threatens violence or harm, or when he or she receives a threat of violence or harm from another student. We will be using the following procedure to inform you if your student either receives or makes a serious threat of violence or harm.

If your son or daughter’s name appears on a list threatening violence or harm, or if he or she receives other threats of violence or harm by another student, we will attempt to meet with you personally, or talk by telephone, within 12 hours of learning about the threat. We will tell you about the threat, who made it (if we know), and the action we are taking to respond to it. We will also send you a notice within 24 hours, stating that you son or daughter did receive a threat. Our number one priority will be to make sure that school is safe for your children.

If your son or daughter made the threat, we will let you know that he or she made it and the action we are taking, including informing the parents of the student who received a threat that your student made. We are also required by the district’s Student Rights and Responsibilities Handbook to inform the police.

Bullying, Harassment and Discrimination Prevention Process (español)

Eugene School District 4J has policies and programs to prohibit and prevent bullying and harassment, including intimidation, cyberbullying, hazing and teen dating violence. Discrimination, meaning any act that has the purpose or effect of unreasonably differentiating in treatment in any instructional program, extracurricular activity, or in the provision of any other service or benefit, is prohibited on the basis of race, color, religion, sex, sexual orientation (actual or perceived), gender identity or expression, national origin, marital status, age or disability.

The district encourages students, parents and other patrons to share their concerns with district officials if they believe they or someone else has been the victim of bullying and harassment, including intimidation, cyberbullying, hazing and teen dating violence, or discrimination. No reprisal or adverse action will occur as a consequence of initiating a complaint in good faith.

Incidents should be reported to the school principal or other staff member. If the complainant has worked with the school and is not satisfied with the resolution, or if the principal is believed to be involved, a complaint may be directed to the superintendent’s office.
