Registration

Parents – Please read and complete the forms below, and bring them with you to Registration Night.

  • CHS Sports Registration Form: Complete for EACH SEASON.
  • Athletic Eligibility Form: (Updated 2/20/2015) Complete for EACH SEASON (Will NOT be kept on file for future sports, MUST be repeated even if your student registered and was eligible during the previous season.)
  • Rules & Code of Conduct: (Updated 2/20/2015) Complete for EACH SEASON (Will NOT be kept on file for future sports, MUST be repeated even if your student registered and was eligible during the previous season.)
  • Physical Examination (English) (Español): You must provide proof of a physical exam every 2 years. Your physical must not expire before the end of the sports season. Students with an expired physical will not be allowed to participate in any athletic activities, including practices.
  • OSAA Academic Requirements: This is a link to the Academic Requirements section of the OSAA handbook for all athletes. Churchill verifies that the student is passing at least 4 classes, and/or passed at least 4 classes during the previous term.
  • Private Auto Insurance Form: Parents that drive students must complete this form. We also recommend registering as a volunteer.
  • Free & Reduced Lunch Application: Families who qualify for free or reduced-price meals may also qualify for a reduced athletic fee.

Seasonal Sports Costs

ASB Fees: All students must have an ASB card to participate in athletics at Churchill. Athletic Fees do not include the $30.00 ASB membership fee.

Cut Sports: All fees are paid up front. Checks/Money will be held for cut sports until teams are finalized. Checks/Money will be returned to students cut from the team.

Sports Reduced Fee Qualifications: In order to qualify for the reduced fee you must provide the free and reduced determination letter. Club sports may not offer a reduced fee.
